LIVING WELL WITH CHRONIC CONDITIONS PROGRAM
A group workshop designed to increase self-management skills and address specific concerns and goals for people with ongoing health problems. Participants set goals and make a step-by- step plan to improve their health. Workshops are interactive and focus on peer support. A resource book is provided for class participants.
The Living Well with Chronic Conditions workshop focuses on key self-management skills such as problem solving, decision making, and goal setting, as well as topic areas such as exercise, healthy eating, managing pain, communication strategies, dealing with emotions, mindfulness, breathing techniques and the mind-body connection.
Living Well with Chronic Conditions is for anyone who:
- Is 18 or older with long-term health conditions such as arthritis, diabetes,asthma, high blood pressure, heart disease, cancer, chronic pain, anxiety,depression, and others
- Feels limited in their day to day activities
- Feels tired, alone, or fearful
- Is looking for better ways to manage symptoms
Workshop sessions take place 1x/week for six weeks. Each session is 2.5 hr. in length. Classes are facilitated by a pair of trained leaders.
No different versions are offered in SE MN at this time.
Most workshops are offered on a free-will donation basis.
